Update My Application
Do you have a change in income, number of family members, mailing address or phone number?
Please complete the Application Update Form and provide GHA with your current information. Please provide copies of documentation evidencing the requested change. If you have any questions while completing the Application Update Form, please call our office and our staff will be happy to assist you and answer any questions you may have.
To return the application change form you may either email at info@glastha.org, mail to 25 Risley Road, Glastonbury, CT 06033 or drop off the documents in person from 9 am - 3 pm Monday-Friday. Please be certain to include the application change form and back up documentation. If you need GHA to make copies of the documents, please come to our office in person during normal business hours. No appointment is necessary.
